In the same or similar clinical situations there are a numerous treatment options exist. Gum colored composites expand the possibilities for clinical and laboratory work. In order to achieve maximum efficiency a correct planning and registration of the individual characteristics of the patient needs to be done.
Aim: Demonstration of the capabilities of gum colored composites.
Materials and methods: Using applied by us, clinical and a mixed clinical-laboratory methods the qualities of gum colored composites are demonstrated. Gingival shade determination and it’s reproduction. Advantages of the presented techniques are shown and main requirements towards the clinical and laboratory techniques are discussed.
Results: The presented, completed and followed clinical cases, show an example of the qualities of the finished restorations.
Conclusion: Gum colored composite resins are an invaluable aid in clinical cases with extreme atrophy in prosthetic field.

Acrylic dentures are not always well accepted by the patients. The thermoplastic materials are method of choice in these cases.
Aim: Fabrication of dentures with thermoplastic materials (TPM) in cases of partial and full edentulous jaws.
Materials and methods: The dentures fabricated with TPM are flexible and elastic, with solid and compact structure, tough and stable and they are a match for the acrylic dentures. The dentures fabricated from flexible materials possess the following advantages:
-
No residual monomer;
-
Eliminate the preconditions for allergic reactions;
-
Dentures lie well on the denture bearing area;
-
No polymerization shrinkage;
-
The flexible resin acts as a shock-absorber and provides softer transmission of the masticatory forces.
Results and discussion: TPM are not a product of chemical reaction during the laboratory stage. They are submitted to physical changes during heating, they are being liquefied and injected under pressure in a preheated flask.
Conclusion: They have high aesthetic values, offered in different shades, transfer the occlusal load much softer, short adaptation period and they are well accepted by the patients.

The tests for assessing the pulp condition conducted immediately after teeth injuries and fractures may show false results. The nervous system may remain reactive, although the surrounding tissues are degenerated. In other cases a temporary loss of sensitivity can occur, while the microcirculation of the pulp is preserved.
Aim: To evaluate the changes in the microcirculation of the pulp of teeth with injuries and fractures by pulse oximetry (PO) and electric pulp test (EPT).
Material and methods: To realize the aim 89 teeth of 31 patients aged from 7 – 35 years are tested. Patients are divided in two groups – up to 18 years and 19-35 years old. Pulse oximetry is conducted with device “Contec CMS 60D”. Electric pulp test was conducted with device “Yonovit.”
Results:
Results in group I:
Average values of EPT: 14,64 μA.
Average values of PO: 87, 35%.
Results in group II:
Average values of EPT: 15,48 μA.
Average values of PO: 87, 74%.
Examination by PO and EPT of a control group of students fifth year was done. It was established that the average saturation is 82, 57% and the average EPT value is 5 μA.
Conclusion: There are changes in the hemodynamics of the pulp of teeth with trauma and fractures that can be established successfully by pulse oximetry.

Trauma of temporary and permanent children’s teeth is unpredictable and relatively common event during childhood. Trauma of child’s permanent tooth is dental emergency which requires immediate professional help. Not every dentist feels confident enough to provide appropriate dental care in such situation. There are cases of incompetent emergency dental treatment provided and follow up care. In Bulgaria there are no sources of information for helpful tips and advices to a child with trauma and his parents.
Aim: To develop interactive software guide for diagnosis and treatment of oral and dental trauma in childhood. We announce the development of our interactive software guide by presenting a selected part – “Avulsio and replantatio.”
Materials and methods: The subjects of visual and textual representation are all dental traumas according to the classification. The material will be presented through a web-based software server of the Medical University of Sofia. Its development was funded by Council of Medical Science Medical University – Sofia (project № 321 / 01.15.2015).
Results: The content of software guide includes all known dental traumas and presents their аetiology, diagnostic criteria, first aid, principles of treatment, instructions for patient and follow up, presented in text, photographs, radiographs, flowcharts. A special part of the guide presents real clinical case, their treatment and follow-up, a total of 150 cases. Some of the clinical cases are presented as test exercise.
Conclusion: The interactive guide is designed for undergraduate students, postgraduate students and dentists.
Keywords: replantation, avulsion, permanent teeth, dental trauma

Autistic spectrum disorders represent a complex set of related developmental disorders characterized by impaired communication, social interaction and repetitive behaviors. They are seen as a major health and educational problems affecting many areas of daily activities, including nutrition.
Aim: The objective of the study is to analyze the eating habits of children with autism and to assess the risk of developing caries in them.
Material and methods: The study comprises 30 children with autism. Assessed their eating habits after analyzing the nutritional diary reflecting diet for seven days – the quantities of protective foods for oral health, relationships between basic foodstuffs and type of food intake, as intermediate snack.
Results: The obtained data showed that 35.2% of children is established a sensory sensitivity, which is the reason for selectivity to food. Carbohydrate diet which is a risk for development of caries, is a serious problem in 50% of children. They have highlighted a preference for soft carbohydrate foods that are present in all their food intake for the day, which will affect their oral risk profile. Quantities of food intake is not sufficient for protection of teeth from decay.
Conclusion: Half of the studied children are at high risk for development of caries.
It is necessary to change their eating habits, taking conducted motivation and education of children and their parents about healthy eating.
Keywords: autism, caries risk, nutrition, sensory sensitivity.

Despite their outstanding characteristics, nickel titanium instruments can unexpectedly fracture. Manufacturers are constantly trying to improve their fracture resistance utilizing new manufacturing techniques and novel movement kinematics.
Aim: The purpose of this study was to analyze lifespan of the size 25/.06 TF instruments, used in continuous and reciprocal rotation and of the size 25/.06 TF Adaptive files utilizing the Adaptive Motion Technology.
Material and methods: One hundred and fifty artificial canals, divided in 3 equal groups, were shaped with twisted files used in different modes of rotation – continuous, Adaptive Motion Technology and reciprocating. After a preliminary creation of a glide path, the shaping was performed with #25/.06 TFs and #25/.06 TF Adaptive files. All instruments worked till their separation occurred. Average lifespan and cumulative survival at the time of all shaping files were examined.
Results: The registered average lifespan of a single file in the first two groups was as follows: continuous rotation – 12.67±4.82; Adaptive Motion Technology – 14.67±5.84 and the difference was insignificant. In the 3rd group average lifespan was not calculated as the second used file did not break till the end of the experiment.
Tested files manifested high survival rates, especially in the group with reciprocation. Cumulative survival in the examined groups revealed no significant difference.
Conclusion: The achieved results demonstrate that movement kinematics affects lifespan of TFs. Reciprocal rotation extends lifespan of tested instruments, although differences are not significant. Mode of rotation should be considered an important factor determining the failure resistance of NiTi rotary files.
